MODEL PICTURE RELEASE FORM
Any model under the age of 18 years old should have a parent or legal guardian present during the photo session, and or must sign, the form below. All models under the age of 18 years old MUST bring an adult with them to any photo session and the accompanying adult must be present during the entire photo shoot/session.
The photographer will not sell/trade/swap the negatives/originals or copies of any photos taken to anyone. All photo’s taken by the photographer (Al McLean) will be copied onto a “CD” and mailed to the “model” usually within 3 to 10 days after the photo shoot (mailed via Canada Post).
If the "model" uses any or all of the pictures taken during the photo session (includes cropped pictures), for any purpose, the model is required to ensure that the photographer "Al McLean" is or has been recognized for being the photographer of the picture or pictures.

Under NO
circumstances will the" photographer" knowingly agree too, or take
any photo's of anyone under the age of 18 years old, that are of nudity in
content.
Cost to the "Model" for this
photo session are agreed to be $ _________

It is mutually
agreed upon if "Al McLean" uses any of the photo's taken on this
date, for any web page or other portfolio listed under the name of "Al
McLean" (this includes different user names on various web pages) , and
the model wishes not to have a particular photo used on any of "Al
McLean's" portfolio's, once notified, "Al McLean"
will remove such photo and may replace it with a different photo that the
"Model" agrees to having posted on such portfolio web page or
such. No frontal topless pictures will be posted on any of
"Al McLean's" portfolio web pages.
If I the
"Model" have my picture taken with one or more other models, I
knowingly accept the fact the other model or models may also receive pictures
(and use those pictures) that I have been photographed in. Also if I work
with another model or models, my "cd" may not be mailed to me for up
to 10 to 15 days from date of the photo shoot.
I the "Model"
hereby release, discharge and agree to save harmless [photographer], his/her
heirs, legal representatives or assigns, and all persons functioning under
his/her permission or authority, or those for whom he/she is functioning, from
any liability by virtue of any blurring, distortion, alteration, optical
illusion, or use in composite form whether intentional or otherwise, that may
occur or be produced in the taking of said picture or in any subsequent
processing thereof, as well as any publication thereof, including without
limitation any claims for libel or invasion of privacy.
I the "Model"
hereby affirm that I am over the age of majority and have the right to contract
in my own name. I have read the above authorization, release and agreement,
prior to its execution; I fully understand the contents thereof. This agreement
shall be binding upon me and my heirs, legal representatives and assigns
